Clinical Nurse (Telehealth – Aged Care)

About the role

Join HomeMade’s telehealth team to support older Australians staying safe and independent at home. You will triage clinical concerns, provide expert assessments, and guide customers and families through informed decision‑making.

Key responsibilities

• Monitor and triage the Clinical Inbox and wellbeing notes.
• Complete clinical assessments and reviews such as VATs and CNAs.
• Support customers and families with informed decision‑making.
• Guide palliative care and advance care planning.
• Respond to incidents and clinical escalations.
• Contribute to continuous improvement and clinical quality initiatives.
• Provide clinical input across the broader team and provider network.
• Ensure practice aligns with the Aged Care Act, Quality Standards, and Clinical Governance Framework.

Required profile

• AHPRA registration as a Registered Nurse.
• Current First Aid certification.
• Experience in home and community care, preferably with aged‑care clients.
• Strong clinical decision‑making and case‑management abilities.
• Warm, human approach with a genuine passion for empowering others.
• Excellent communication and customer‑service skills.
