Hospital Pharmacist – 12‑Month Fixed‑Term Role

About the role

We are seeking a qualified clinical pharmacist to join Epic Pharmacy’s team at Northern Beaches Hospital on a 12‑month full‑time fixed‑term contract. The role involves delivering high‑quality pharmacy services to both inpatient and outpatient populations, supporting the safe and effective use of medicines.

Key responsibilities

• Provide efficient clinical pharmacy services across hospital wards.
• Conduct medication history interviews, medication reconciliation and pharmaceutical reviews at transfers of care.
• Dispense and distribute medication accurately and promptly.
• Perform therapeutic drug monitoring, manage medication outcomes and adverse drug reactions.
• Offer medication counselling and education to patients.
• Collaborate with multidisciplinary teams and advise on patient care.
• Participate in training sessions for pharmacy, nursing, medical and allied health staff.
• Contribute to quality‑use‑of‑medicines projects, audits and committees.
• Consider financial implications of medication use for hospital and community patients.
• Support on‑call and weekend pharmacy services.

Required profile

• Registered pharmacist with clinical experience in a hospital setting.
• Proven ability to conduct medication history, reconciliation and reviews.
• Experience in dispensing, therapeutic drug monitoring and adverse reaction management.
• Strong communication skills for patient counselling and multidisciplinary collaboration.
• Willingness to engage in training, audits and on‑call duties.
