Accommodation Coordinator - Housing & Accommodation

About the role

St Jude's Health Care Group, a provider of aged care, allied health and disability services, is looking for an Accommodation Coordinator. The role aims to maximise the quality of life of clients with disability or psychosocial needs by making sure the right support services are delivered to people living in its accommodation across the metropolitan area. It is a full-time position, Monday to Friday, 9am to 5pm.

Key responsibilities

• Manage the day-to-day operations of the houses in your area, including rostering and supervising Support Worker staff.
• Work from the accommodations to supervise and witness the delivery of services to clients.
• Complete client assessments, care planning, individualised support plans, positive behaviour support plans and other relevant assessments and reviews.
• Ensure services match each client's funding and preferences so they remain cost-effective and person-centred.

Required profile

• Previous experience in case management or support coordination within the disability sector.
• Familiarity with the NDIS framework and terms of business.
• Ability to build lasting relationships with clients and the staff you supervise.
• Superb communication skills and confidence talking to a wide range of stakeholders.
• Values feedback and works well within the coordination team.
• Possesses or is willing to obtain an NDIS Worker Screening Check and a Working with Children Check.
• Holds a valid WA licence and a reliable vehicle.
