Registered Nurse – LGBTQIA+ Community Supports (Sydney CBD)

About the role

Chosen Family is seeking a compassionate Registered Nurse to deliver inclusive, trauma‑informed nursing care to LGBTQIA+ participants across disability and community settings in Sydney CBD. The role focuses on providing safe, affirming health support for individuals who may have faced discrimination, complex trauma, or barriers to healthcare.

Key responsibilities

• Conduct comprehensive nursing assessments and health reviews.
• Provide complex clinical supports such as medication administration, PEG feeding, wound care, catheter management, diabetes and seizure management, mental health monitoring, continence care, and behavioural observation.
• Monitor participant wellbeing, escalate concerns, and coordinate appropriate interventions.
• Develop and review participant care plans, offering clinical oversight to support workers and ENs.
• Participate in multidisciplinary case conferences, safeguarding processes, and risk‑management activities.
• Liaise with families, guardians, allied health professionals, and LGBTQIA+ inclusive services.

Required profile

• Bachelor of Nursing.
• Current AHPRA registration as a Registered Nurse.
• Current NDIS Worker Screening Check.
• Demonstrated empathy, cultural responsiveness, and commitment to inclusive, person‑centred care.
