Hospital Pharmacist – Northwest Hospitals

About the role

The Northwest Hospitals Pharmacy Department is seeking a qualified pharmacist to join its team delivering comprehensive pharmacy services to inpatients and outpatients across the North West Regional Hospital in Burnie and Mersey Community Hospital in Latrobe.

Key responsibilities

• Dispense medications for inpatient, outpatient and discharge supply in compliance with all legal requirements and hospital medication access schemes.
• Provide verbal and written medicines information and health advice to patients.
• Offer medication information and education to hospital staff and support their ongoing learning.
• Conduct Best Possible Medication History taking and develop medication management and action plans with the multidisciplinary team.
• Participate in the Advanced Pharmacy Australia Resident Training Program, gaining clinical, operational and professional development experience.

Required profile

• Pharmacy experience in dispensing or clinical pharmacy, preferably within a hospital setting.
• Knowledge of relevant legislation, codes of practice and the Pharmaceutical Benefits Scheme.
• Ability to assess drug regimens, provide tailored medicines information and collaborate with multidisciplinary teams.
• Commitment to patient safety, continuous learning and professional development.
