Palliative Care Pharmacist (Part‑time)

About the role

This permanent, part‑time position supports the newly established palliative care unit at Mater Hospital North Sydney. As the first dedicated pharmacist for this service, you will work within Epic Pharmacy to enhance medication management for patients with complex therapeutic needs.

Key responsibilities

• Conduct medication reconciliation and comprehensive medication reviews.
• Optimise symptom‑management regimens, including pain control and end‑of‑life care.
• Provide advice on deprescribing and safe use of specialist palliative medicines.
• Collaborate with medical, nursing and allied‑health teams to ensure seamless patient care.
• Educate patients, carers and staff on medication use and best practices.
• Facilitate transitions of care between hospital, community teams and other facilities.

Required profile

• AHPRA‑registered pharmacist with no practice restrictions.
• Australian working rights for at least 12 months from start date.
• Demonstrated experience in clinical pharmacy services such as medication review or reconciliation.
• Strong interest in patient‑centred care within complex or supportive‑care settings.

Required skills

• Clinical pharmacy expertise in medication reconciliation and review.
• Knowledge of palliative‑care pharmacotherapy, including pain management and deprescribing.
• Ability to work collaboratively within multidisciplinary teams.
