Discharge Nurse – Part‑time (Permanent)

About the role

The Discharge Nurse will join St John of God Bunbury Hospital on a permanent part‑time basis. Working closely with the Discharge Coordinator, nursing managers and multidisciplinary teams, you will ensure safe, timely and coordinated discharge planning for patients with complex needs.

Key responsibilities

• Coordinate discharge planning for patients identified as requiring a Discharge Planner.
• Initiate early discharge planning aligned with appropriate length of stay.
• Liaise with internal and external health professionals to facilitate effective discharge.
• Inform and involve patients, carers and families in the discharge process and available support options.
• Communicate with community service providers for “at‑risk” patients needing multiple services.
• Assist nursing management with length‑of‑stay and patient‑centred care coordination.
• Maintain accurate, confidential records and contribute to data collection for discharge services.
• Participate in quality improvement activities and continuous evaluation of discharge processes.

Required profile

• Current registration as a Registered Nurse in Australia.
• Experience in acute hospital settings, preferably with discharge planning or case management.
• Strong understanding of multidisciplinary collaboration and patient‑centred care.
• Excellent communication skills with patients, families and external service providers.
• Commitment to confidentiality, professional development and quality improvement.
